This workshop will introduce the opportunity for VCS organisations to be a part of the forthcoming Diabetes Prevention Contract for Bradford City and District CCG areas.
This workshop will explore the potential opportunity to be part of a VCS offer as part of the Extended Access Service in the North and South Areas of Bradford starting on 1st October 2018.
The South 10 (S10) Community Partnership are keen to work in partnership with local VCS organisations to delivery 3 services over the next 15 months (January 2020 – March 2021). These are: Communications and Engagement Service Self-Care Champion Role Youth
